NHS Yorkshire and the Humber

The Care Partnerships team is part of the SHA’s Patient Care and Partnerships Directorate.

They help the NHS in the region to work more effectively with partner organisations, especially local authorities, to reduce health inequalities and improve health and wellbeing.

The team supports PCTs to increase their capacity and capability to become more effective commissioners of care services in the areas of:

  • children and young people 
  • drug and substance misuse
  • learning disabilities
  • maternity
  • mental health
  • NHS continuing care
  • offender health (including prison health)
  • older people

They also manage the performance of PCTs around the delivery of national and local policies and priorities and a better user and carer experience of services - through work with other SHA departments, linking where appropriate with the Department of Health, participation in local networks and direct contact with NHS organisations.

The care partnerships team liaise with other regional organisations to improve co-ordination of joint work on health and wellbeing and to act as a link between regional agencies and the local NHS where appropriate.
